Your Opportunity

Located in Blacktown, Homelessness outreach prevention and rapid rehousing service - HOPARRS is a Specialist Service that works to prevent and reverse homelessness for men women and families through the provision of client centred case management support, assertive outreach, advocacy, referrals and advice.  A key focus is early intervention, rapid rehousing and ongoing support to enable people to remain in their homes in the Blacktown, Parramatta and Holroyd LGA’S.   

 The role of Case Worker is to support clients experiencing homelessness or at risk of homelessness to establish or sustain tenancies.

Your key responsibilities will be to:

  • Respond to referrals of clients to the service from internal and external support services
  • Conduct over-the-phone and formal face-to-face assessments of suitability for support
  • Undertake initial registrations for clients, including all necessary paperwork
  • Ensure accurate client information and record keeping
  • Create individualised case plans including referrals to supplementary services
  • Provide intensive ongoing case management sessions (formal and informal) with clients
  • Develop strong relationships with key external stakeholders including other services
  • Provide outreach services at key first to know agencies, e.g. Housing, Centrelink
  • Oversight of a men’s transitional accommodation property

Requirements for Success

  • A Degree or Diploma in Social Welfare/Social Science or the related humanities
  • Experience in case management and knowledge of the homelessness sector
  • A current NSW Driver’s License
  • Senior First Aid Certificate
  • Working with children check in NSW

If you are successful, you will be required to undertake a National Criminal History Check initiated by us if you haven’t completed one in the last 12 months.
