Your Opportunity

In this role you will assess and respond to the needs of young people who have been referred to the TYSS service for individual, intensive case management support within a therapeutic framework.  Your key responsibilities will include:

  • Respond to referrals using a strengths based client centered approach when assessing the support needs of the young person and where necessary their parents
  • Develop, implement and review individualised case plans for each client, addressing such issues as living skills, welfare needs, interpersonal skills, health, education, training, employment options, social skills, recreational needs and family relationships including making appropriate referrals
  • Schedule and conduct ongoing intensive case management sessions with the young person (and their parents where necessary) that is client centered and goal focused, in a therapeutic framework
  • Use therapeutic tools, support, information and education to assist the young person to meet their identified goals
  • Develop quality relationships with a range of key stakeholders and partners including community organisations and other health professionals to allow for the effective provision of support and information to the client
  • Advocate and liaise on behalf of the young person in a range of forums including DHHS, Department of Education and Child Safety Services
  • Monitor risk levels and provide unscheduled support where possible, to young people in times of crisis including over the phone support
  • Conduct ongoing assessment of the needs of the young person to determine the continuing need for intensive case management and therapeutic interventions and referral to other services as needed

Position Requirements

To be successful in this role, you will need to possess:

  • Extensive experience in supporting disadvantaged youth to achieve positive outcomes
  • Previous experience in intensive therapeutic case management and client centered support
  • Experience in establishing and maintaining community networks and therapeutic relationships to enhance service delivery
  • Strong understanding of contemporary youth issues including homelessness, poverty, abuse, mental health and the use and misuse of alcohol and other drugs, and knowledge of support services available to address these issues
  • Relevant tertiary qualifications and experience in Youth work or related field
  • Senior First Aid Certificate or willingness to obtain
  • Current Driver’s License

As part of our Employment screening process you will be required to undertake a National Criminal History Check.
