About the role

The Youth Accommodation and Support Service (YASS) provides support, assistance and accommodation for young people who are experiencing or at risk of homelessness. The service provides support to vulnerable young people by connecting them to school, work, vocational training and other support services whilst assisting them to obtain long term housing.

The role of the Case Manager will be to work within a Trauma Informed Care framework  to provide holistic support to young people living on site in crisis and transitional accommodation through the development of individual goal and case management plans. To be successful in those role, you MUST have experience working within the Youth Sector in a Case Management role as well as experience in working in youth residential settings. Duties:

  • Develop, implement and review individualised support plans for each young person in line with their treatment goals;
  • Work within a Trauma Informed Care framework as well as the WA Youth Work Code of Ethics;
  • Provide a holistic approach to case management by providing timely advocacy and referrals;
  • Respond to and manage crisis situations;
  • Build links in the community to ensure young people have every opportunity of success; and
  • Provide general daily program support.

Qualifications & Experience:

  • Prior relevant experience and sound understanding of issues facing youth and homelessness
  • Demonstrated experience working with young people with high and complex needs
  • Relevant Youth Work qualification
  • Demonstrated compliance with the Code of Ethics for Youth Workers in WA
  • Current Senior First Aid Certificate or willingness to obtain
  • Be computer literate in Microsoft programs
  • Be willing to provide on call support where required as part of a roster
