About the Program/Role

The Personal Helpers and Mentors Program (PHaMs) assists people who have a diagnosed, severe functional limitation resulting from a mental illness to better manage their daily activities and to successfully transition and access the National Disability Insurance Scheme (NDIS) in their local region.

We currently have a 2 vacancies for Case Managers to join our team in Alice Springs on a full time, fixed term basis until 30 June, 2019.

Duties

  • Respond to referrals and assess against access eligibility
  • Undertake initial assessments of clients and support their application and successful transition into the National Disability Insurance Scheme (NDIS)
  • Develop strong relationships with key stakeholders across Alice Springs & Papunya
  • Provide effective and recovery based approach case management
  • Create and maintain up to date individualised case management files and recovery plans
  • Complete a range of internal and external reports
  • Work in the Papunya community strengthening relationships with current & potential participants and working together within the community and local stakeholders which includes overnight stay in community
  • Attend local community meetings, forums and community awareness events, training and conferences as directed

Qualifications & experience:

  • Minimum Certificate IV in Mental Health or Community Services
  • Demonstrated 3 years’ working experience in mental health or similar
  • Demonstrated experience working with Aboriginal and Torres Strait Islander communities
  • Excellent Computer skills including Microsoft Office
  • Demonstrated excellent communication skills, verbally and written
  • Ability to travel overnight regularly to remote communities in the NT/Alice Springs region
  • Current NT driver’s license
  • Current NT WWCC (OCHRE Card)

In addition there is a requirement to possess a current Working with Children check or apply if you do not already hold one.
